Direct Support Professional II
Take Another Chance is seeking an experienced Direct Support Professional (DSP) to join our team. DSPs provide hands-on support to individuals with developmental disabilities in a 24-hour awake care residential setting, helping them achieve greater independence, community involvement, and quality of life.

This role includes assisting with activities of daily living, medication administration, transportation, community integration, and the implementation of individualized support plans.
This is a part-time position with scheduling based on client needs and service requirements.
Key Responsibilities
Personal Care & Daily Living Support
- Provide person-centered support with activities of daily living, including bathing, grooming, toileting, dressing, and meal preparation.
- Assist individuals with mobility, transfers, and physical support needs while promoting dignity and independence.
- Encourage skill development and increased independence through daily routines and individualized supports.
Health, Safety & Medication Support
- Administer medications in accordance with Ohio DODD standards, physician orders, and agency policies.
- Monitor individuals for changes in health, behavior, or overall well-being and communicate concerns promptly to appropriate team members.
- Implement and follow Individual Service Plans (ISPs), behavior support strategies, and safety plans to ensure quality care.
Home & Community Integration
- Support individuals in participating in community activities, appointments, recreational opportunities, and personal interests.
- Provide safe and reliable transportation as needed.
- Maintain a clean, organized, and safe living environment that supports the individual's health and well-being.
Documentation, Communication & Compliance
- Complete accurate, timely, and objective documentation, including daily notes, medication records, and incident reporting as required.
- Maintain compliance with company policies, ISP requirements, DODD regulations, and all applicable standards.
- Participate in team meetings, trainings, and shift communication to support continuity of care and consistent service delivery.
Relationship Building & Team Collaboration
- Develop positive, professional relationships with individuals served, families, guardians, and interdisciplinary team members.
- Foster an environment of respect, dignity, choice, and self-advocacy.
- Collaborate effectively with coworkers to ensure consistent supports and high-quality services across all shifts.
- Serve as a positive role model and resource for newer team members when appropriate.
Requirements
- Must be at least 18 years old
- High school diploma or equivalent
- Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles
- Reliable daily access to a personal vehicle to transport clients
- Proof of active auto insurance meeting Ohio minimum requirements
- Ability to pass both a criminal and a driving history background check
- Current AHA Heartsaver First Aid/CPR/AED certification
- Current Category I Medication Administration Certification with the Ohio DODD
- Minimum 2 years of prior experience supporting individuals with developmental disabilities
- Ability to lift up to 50 pounds and perform physical tasks, including bending, reaching, and standing for extended periods
- Ability to work assigned shifts consistently to support continuity of care
Professional Skills & Competencies
- Proficiency with electronic documentation systems and basic computer applications
- Strong written and verbal communication skills, including accurate and timely documentation
- Ability to work independently while exercising sound judgment and decision-making
- Professional interpersonal skills to effectively support individuals served and collaborate with team members
- Strong understanding of person-centered supports, client rights, and maintaining professional boundaries
- Ability to adapt to changing client needs while maintaining a calm, professional demeanor
- Demonstrated reliability, accountability, and commitment to quality care
Preferred Qualifications:
- 3–4 years of experience supporting individuals with developmental disabilities
- Current Ohio DODD Category II and/or Category III Medication Administration Certification
- Experience implementing Individual Service Plans (ISPs) and documenting progress toward outcomes
- Experience supporting individuals with behavioral support needs, personal care needs, or community integration goals
- Experience training, mentoring, or serving as a resource for newer Direct Support Professionals
- Proficiency with electronic documentation systems and Microsoft Office Suite
